The questions on this application are necessary to provide for the best interest of the pet and the adoptive family. Completion of this application does not guarantee adoption of any pet. References will be checked. The Fairfield Area Humane Society has the right to decline any application based on the information provided herein. Some adoptions may require a home inspection before final approval. By filling out this application you agree to all FAHS requirements.
